چکیده

T HIS 56-YEAR-OLD MAN WAS ADMITTED to Hahnemann Hospital because of frequent Adams-Stokes attacks. He had experienced an acute myocardial infarction in 1958 and again in 1961. Frequent convulsions occurred in spite of administration of atropine, isoproterenol, and chlorothiazide. On January 3, 1962, an electric pacemaker was implanted on the left ventricle. The following electrocardiographic strips are presented to illustrate an unusual combination of rhythms following the implantation of an electric pacemaker and to demonstrate the absolute refractory period of the human ventricle. 1. On admission (A) sinus tachycardia was present at l40/min. The ventricles are under control of a ventricular pacemaker (30/min.). Failure of the subsidary pacemaker engendered periods of asystole and Adarns-Stokes attacks. Following the insertion of the pacemaker (B) a regular ventricular response (60/min. ) was present. The atria are under control of the sinus node ( 100/min.) while the ventricles are entirely controlled by the electrical pacemaker.

Pacemaker Twiddler's Syndrome

A 76-yr-old man with unipolar ventricular pacemaker, previously implanted in the right ventricular apex, presented with syncopal attack and twitching of the right pectoralis muscle. He finally was diagnosed as having Twiddler's syndrome. The patient had a pulse rate of 36/min and cannon waves in the neck veins. Chest radiography showed a dislodged pacing lead completely wrapped around the pulse...

Life-Threatening Effect of Twiddler's Syndrome with a Subpectorally Implanted Cardioverter Defibrillator

Twiddler's syndrome has been known for over 30 years as a rare complication after pacemaker implantation [1]. The often unintended manipulation of the pacemaker by patients causes the device to rotate in the pacemaker pocket, which may result in lead defects or dislocations. The same mechanism exists for transvenously implanted defibrillators and has been described as a rare complication for ab...

THE ROLE OF PACEMAKER CLINIC IN THE FOLLOW-UP OF PATIENTS WITH PERMANENT PACEMAKERS

The Pacemaker Clinic at Shahid Rajai Cardiovascular Research Center first started operating in August 1992. In its first year of operation, 294 permanent pacemaker patients were studied, allowing an appraisal to be carried out of the clinic's effectiveness in diagnosing and treating early complications. In the final analysis, pacemaker complications were observed in 68 (23%) patients, of wh...

Peritoneal migration of an abdominally implanted epicardial pacemaker: a cause of intestinal obstruction.

We report the case of a 10-year-old child with an abdominally implanted epicardial pacemaker that eroded through the peritoneum and migrated to an intraperitoneal location, resulting in partial and then complete intestinal obstruction. This potentially life-threatening complication should be considered when a patient with an abdominally implanted pacemaker presents with abdominal pain.

Pacemaker implantation complication rates in elderly and young patients

AIMS To evaluate the complication rate differences between elderly and younger patients who receive a permanent pacemaker implantation. METHODS We reviewed all cases admitted to our institution between January 2008 and June 2009 with symptomatic bradyarrhythmia for whom a permanent pacemaker was implanted. Beginning in June 2009, we prospectively collected data from all patients with the same...
